Tried it on my system at home.  Booted into DOS using real mode scsi card drivers.  The hardware I used was as follows:
Sound Blaster AWE64
Tekram DC390U SCSI Card
Plextor 40X SCSI cdrom

When trying to play audio off of an audio cd with MPXPlay, I couldn't get it to work either.  All I got was a bunch of noise.

I was able to get it work, though, using FreeCDP:

I think we may be running into a limitation in MPXPlay itself.

Might wanna look into using a separate app for playing audio cd's, then come up with a way to switch between the two.

The following page links to a number of DOS-Based sound programs:

Hope this helps.

Does anyone know of a way to incorporate a SB16 SCSI sound card with MPXPLAY? I get great sound off the hard disk, but no sound from the CD. I can get sound off the CD if I use the Adaptec CDPLAYER program that comes with the cards drivers. Any ideas? (other than switching sound cards).
